Family TIES of Nevada provides training, information and referral, emotional support and advocacy to people who have disabilities, their family members and the professionals that work on their behalf. If you know someone who could benefit from our services, print out this form (English and Spanish) and fax the completed, signed form to us at 775-323-2205, or scan it and email it to info@familytiesnv.org.
By providing the following information, you are giving Family TIES of Nevada permission to initiate contact with the person you are referring for services. Upon receiving your referral, the person will be contacted within 48 hours in most cases, or immediately for urgent matters. All information is treated as confidential and will not be released to outside organizations or individuals.
